According to the Utah Department of Public Safety, the bulldozer came loose as the tow truck made a right-hand turn, causing the fatal impact. Richard and Sally were traveling in the pickup while towing a boat and were pronounced dead at the scene. Richard’s wife and two other children survived with non-life-threatening injuries. A third daughter was away abroad at the time.
